Job Description

Liable to provide direct patient-centered care and integrated pharmacy operational services in a decentralized practice setting with physicians, nurses, and other hospital personnel. CLINICAL PHARMACIST at KFSH&RC Jeddah design, recommend, monitor, and evaluate patient-specific pharmacotherapy to ensure optimal outcomes. They assure accurate and timely distribution of medications to patients. Work as a clinical resource for other pharmacy personnel, medical and nursing staff, as well as other healthcare professionals.

Essential Responsibilities and Duties

  1. Assists medical staff with the selection of drug therapy and monitoring for a specific patient population.
  2. Process incoming new prescriptions, refills and transfer requests, provide drug information including but not limited to drug selection, dosage, interactions, medication and patient monitoring and side effects, etc. to all health care providers.
  3. Assists and guides healthcare providers with issues related to the medication management and use system.
  4. Reviews, verifies and interprets medication orders for clarity, accuracy, rationality, compatibility, feasibility, and suitability for the patient’s specific needs.
  5. Participates in patient care rounds with other members of the healthcare team as appropriate.
  6. Documents interventions and recommendations/clarifications in the electronic Health Information System as applicable.
  7. Detects, analyses, and reports adverse drug reactions and medication errors.
  8. Supervises, precepts, checks, and monitors medication related activities/performance of lower/junior pharmacists, residents, trainees/ students.
  9. Consults and seeks directions/assistance/guidance from senior pharmacist /manager with difficult /unusual problems, questions, or actions.
  10. Advises/counsels patients and answers questions about medications (including indication, possible side effects, and interactions among different drugs as applicable to the area of practice.
  11. Assures adherence to the departmental policies and procedures and provides monthly reports on clinical activities and other relevant workload statistics.
  12. Assumes responsibility for patient-specific pharmacotherapy plan to assure optimal outcomes.
  13. Actively participates in specialized patient care rounds with other members of the specialized healthcare team.
  14. Participates in the performance improvements, educational, and other clinical activities.
  15. Participates/assists in conducting performance/quality improvement projects.
  16. Follows all Hospital’s related Policies and Procedures.
  17. Participates in self and others’ education, training and development, as applicable.

Education

Pharm.D./Bachelor’s Degree in Pharmacy with accredited residency program (PGY1) is required.

Experience Required

Completion of accredited General Pharmacy Practice Residency (PGY1)

Diploma with Pharm.D./Bachelor’s Degree is required.

Other Requirements(Certificates)

Current Registration from country of origin and Saudi Commission for Health Specialties (SCHS).
